Boarding Reservation Form

  • New customers: All new customers will need to fill out the "Pet Information Form" in addition to the Reservation Form.
  • Required: Frontline Plus must be applied in our office the day they check in. You may bring your own or purchase it from us.
  • Vaccinations: Vaccinations must be current or can be updated by Dr. Burroughs DVM upon arrival. if needed you may fax new updates or email them.
    • Canine: DHLPP, Rabies, Bordetella
    • Feline: FVRCP, Rabies
